Does not work with commands. You could test it if the item is dropped.

It could work with a data pack. Not with raw commands.

If you retexturize and remodel a Carrot on a stick, you can make it look like any item, you then have to rename it using cmds and can read out in the data pack via cmd whether it is just clicked. Then you give the player the strength effect. Of course it would be easier with a fishing rod.
