Iron golems spawn only once per village. If they die, a new one spawns with a low probability.

We play 1.14 and my friend works the farm, we use bells and villager FROM a village that is 500 blocks away.

The maximum radius for something that still counts to the village is at Faith 100 blocks. It does not matter where the villagers or the bell come from, it does not count as a village.
