Everything rumors. Mojang used to write in the update info that they had removed Herobrine (as a little gag they did until Microsoft bought Mojang)

By searching through old patch notes, people found out that it existed for a short time, but was then removed from the game.
As far as I can tell, it must have been 2011/12 when Herobrine was patched in / out.

No, but the 1.16 patch notes don't say -Removed Herobrine, so who knows - maybe it's now. I haven't seen it yet, but it can be.
