Minecraft sword is not shown ingame?

I'm a pack designer and designed a sword. My problem is that it is not shown to me in-game or as pink-black square. This means that the game can't find any files.

Why is that? Should I position it differently or something?

I can only guess now and it's been a while since I made packs, but could it be because you made it in the middle of the picture and not in the lower left corner? If that is irrelevant, I said nothing and unfortunately no other idea.

This is when the item does not correspond to the size of the texture pack itself, so you just have to change this item in either a smaller or larger size (of your respective texture pack)

You can see this by opening other items that are in the type (either with properties or via your editing program), where the dimensions etc. Are detailed. Compare size with your item and then change your item.
