I started a 1.16.5 world with the quark mod (and a few others that don't add any structures) and later added Ice and Fire.
It has no effect on what has already been generated, but Ice and Fire influences new chunks or dimensions that I entered after adding them (i.e. Generated with Ice and Fire)
Yes, the ice and fire structures are generated in the new chunks.
